Introduction. The author would like to thank all those who wrote in after the publication of the first volume of this series with additional information and/or corrections. Rather than wait for a supplement to the final (fourth) volume of this series before collating all the supplementary information that had been acquired in the course of its publication (as was suggested in the "Introduction'' to Volume 1), it has been decided that each successive volume will include an "Addendum" embodying all extra information or corrections relating to the previous volume (or volumes).
